旨在探讨肺动脉吊带合并先天性气管狭窄患儿中与气管成形术相关的解剖及临床因素。本回顾性多中心研究纳入 2010 年 1 月至 2025 年 9 月间在中国三家心脏中心接受手术治疗的肺动脉吊带患儿。回顾术前临床特征、CT 测量数据及手术资料。采用单因素和多因素 logistic 回归分析鉴定与气管成形术独立相关的因素,并进行交互分析。在排除 24 例术前影像资料不完整的患者后,共纳入 330 例肺动脉吊带合并先天性气管狭窄患儿。其中 302 例 (91.5%) 接受了滑动气管成形术,28 例 (8.5%) 仅接受肺动脉吊带矫治术。低龄、长段气管狭窄及更严重的官腔狭窄独立与较高的气管成形术比例相关。该多中心队列中,低龄、长段气管狭窄及更严重的官腔狭窄与气管成形术独立相关。气管成形术的实施似乎受多因素影响,而非取决于任何单一影像学参数。仍需开展前瞻性研究并进行长期随访以评估不同管理策略的临床结局。

To investigate anatomical and clinical factors associated with tracheoplasty in children with pulmonary artery sling and congenital tracheal stenosis. This retrospective multicenter study included children with pulmonary artery sling who underwent surgical treatment at three cardiac centers in China between January 2010 and September 2025. Preoperative clinical characteristics, computed tomography measurements, and operative data were reviewed. Univariable and multivariable logistic regression analyses were performed to identify factors independently associated with tracheoplasty. Interaction analyses were also conducted. After excluding 24 patients with incomplete preoperative imaging data, 330 patients with pulmonary artery sling and congenital tracheal stenosis were included. Among them, 302 (91.5%) underwent slide tracheoplasty and 28 (8.5%) underwent pulmonary artery sling correction alone. Younger age, long-segment tracheal stenosis, and more severe airway narrowing were independently associated with a higher frequency of tracheoplasty. Tracheoplasty performance appeared to be influenced by multiple factors rather than any single imaging parameter. Further prospective studies with long-term follow-up are required to evaluate clinical outcomes associated with different management strategies.
