右肺上叶切除术中遇到的一例罕见肺血管连接

肺血管变异可能增加胸外科手术的复杂性，尤其在电视辅助手术中，此类异常会增加出血风险。本文报告一例 77 岁女性患者，因浸润性黏液腺癌接受右肺上叶切除术。影像学显示右肺上叶后部一个 2 cm 结节，FDG 摄取较低。组织学证实为混合性腺癌和大细胞神经内分泌癌（IA 期）。术中发现并结扎了一条符合动静脉瘘特征的血管连接，位于肺上静脉和叶间肺动脉之间。

Pulmonary vascular variations may complicate thoracic surgery, particularly video-assisted procedures where such anomalies increase the risk of bleeding. We report a 77-year-old female undergoing right upper lobectomy for invasive mucinous adenocarcinoma. Imaging revealed a 2 cm posterior right upper lobe nodule with low FDG uptake. Histology showed mixed adenocarcinoma and large-cell neuroendocrine carcinoma (Stage IA). Intraoperatively, a vascular connection consistent with an arteriovenous fistula between the superior pulmonary vein and interlobar pulmonary artery was identified and ligated.
